AH presents with acute onset severe dyspnea, non-productive cough, low-grade fever, and presyncopal symptoms. She is hypoxic 90% on room air, has a normal heart and lung exam, but has bilateral lower extremity edema, and right sided calf tenderness. Clinical test: CXR shows small left sided pleural effusion, D-Dimer 750, CT pulmonary angiogram shows bilateral central pulmonary emboli with left lower lobe pulmonary infarction.
