AH presents with acute onset severe dyspnea, non-productive cough, low-grade fever, and presyncopal symptoms. She is hypoxic 90% on room air, has a normal heart and lung exam, but has bilateral lower extremity edema, and right sided calf tenderness. AH is s/p right total knee replacement 6 weeks prior, past medical history of obesity, Type II diabetic, Hypertension, 2 pack per day smoker and sedentary lifestyle since surgery. She has a family history of CAD. Clinical test: CXR shows small left sided pleural effusion, D-Dimer 750, CT pulmonary angiogram shows bilateral central pulmonary emboli with left lower lobe pulmonary infarction. The plan is to admit AH to the hospital begin anticoagulant therapy (low molecular weight Heparin). Format of choice 430 word paper.
